Summer Series Entry Form

A separate entry form is required for each entrant.

​If making a single payments for 2 or more entrants:
- make sure all entrants include "Payer's Name" on their Entry Form.
- the Payer must include a list of all people in the group, the amounts being paid for each, and the total amount.
- if posting, submit all the forms together.
